Roles and Responsibilities
- Be responsible for the coordination and management of a daily position statement and circulated daily, or as and when required in times of surge, to all partners.
- To use active listening skills, observation, and communication to build relationships with all teams, partners, and providers.
- To be an ambassador for South Tyneside Social Services promoting good working relationships in line with South Tyneside’s PROUD values.
- To be able to confidently and professionally challenge decisions, delays and queries which impact the flow of hospital discharge.
- To participate in effective solution focussed discussions to manage flow. Escalating to the Hospital Discharge Manager where there are significant delays or risks.
- Ensure there is access to care and support if needed on the day of discharge and work with partners to reduce delays and promote wellbeing.
- To work closely with health, housing, and voluntary sector colleagues to ensure resources are used effectively across the system. Multi-disciplinary working will be critical to the success of this role.
- To work in accordance with safeguarding policies, safeguarding individuals in a way that supports them in making choices and having control. Ensuring that the ‘Making Safeguarding Personal’ ethos is at the heart of all practice.
- To develop an awareness of services available at home and understand the roles and accountabilities of other agencies and professionals in order to refer, liaise, consult, and work effectively in order to meet identified needs.
- Oversight and management of daily tasks, phone calls and email enquiries received into the Hospital Discharge Team.
- Manage daily operational queries including those of a complex or potentially contentious nature, in a way that is customer focussed and lends itself to positive outcomes.
- Ability to maintain resilience while working under pressure in a fast-paced service.
- Ability to work autonomously as well as within a team.
- Flexibility to adapt to change as the service demands.
- Must have due regard to the Council’s current management arrangements for Data Quality. All employees have a responsibility to ensure that the data they collect, manage and report, including data from third parties is accurate, valid, reliable, relevant, complete. Data should be produced in a timely fashion to aid sound decision making and that appropriate procedures, systems and processes are in place to provide quality data.
- To support with the management of data quality assurance as directed by the Hospital Discharge Manager.
- To prepare and present reports of a statutory and non-statutory nature.
- To be open to engage in peer support and challenge.
- Champion a commitment to equality and diversity throughout the service and implement relevant anti-discriminatory policies in all aspects of the service’s work. Ensuring that all aspects of the equality’s legislation requirements are understood and acted upon with the service and beyond as appropriate.
